Legal Question

I was in a car accident 9/15/99 I've had 4 surgeries on my right shoulder. I continue to have pain. Recently I had an MRI and it did not show any issues but I still have pain. My doctor wanted another mri to rule out a re-tear and also PT. The carrier said in so many words it wouldn't help or be neccessary. What are my options?
