Legal Question in Administrative Law in Israel

I need to give general power of attorney to my mother in Israel so she can withdraw funds from my bank account. I know I need to have a power of attorney document notarized and then stamped with the apostille stamp from the state of arizona. Is there a specific document that I need - or can I just find a regular P.O.A. form online and get it notarized? Will I need to have this officially translated into hebrew or is a notarized, stamped legal document good in any country that is part of the apostille stamp program? thanks so much...

The best idea for you might be to sign the POA in font of one of the Israeli consuls in the USA (LA may be the closest location)
