Legal Question

Hi There,

I am a resident of India and I want to change my name from "Mohammed Shoaib" to "Syed Mohammed Shoaib" where "Syed" is my surname. Please let me know in that case will it be a change of name case (or) addition of surname to my name case?

Also please let me know what procedure to follow for the same. Thanks.
