Sir i have lost my DD, pls tell me how to apply for duplicate DD

1) Immediately report to the issuing branch loss of DD with full details to exercise caution against its fraudulent use without delay.

2) Apply to Bank for issue of Duplicate DD by submitting prescribed Affidavit Indemnity Bond on Rs.100/ NJS paper.

3) After confirming that the DD remain unpaid, Bank will issue DD.

Dear Sir / Madam,

You should inform vide an application to the Bank of the Demand Draft having been misplaced and complete the requisite formalities for issue of a fresh Demand Draft.
