Please contact the Surat Municipal Corporation (SMC)to avail all the details as you are supposed to file a affidavit to the Corporation conditions.

It is not difficult to obtain. Contact local revenue authorities. They will provide you forms and tell you about fee. Domicile of father is not necessary. Proof of residence is necessary.

Normally, the domicile of a minor is same as that of his father, unless there is an exception in the Rules framed by Maharashtra government.
