Legal Guardianship

My 55 years old brother with bipolar and schzophrenia disorders for the last 35 years, needs a legal guardian to help him with his legal rights in regards to his housing, medications, doctors, social security benefits, and any money left to him after our mother passes away. I am his sister acting on his behalf for the last two years after my father died, I need a legal form for my brother and I to sign stating I am legal guardian. Where do I find such a form?

You can download forms from the Superior Court website "www.superiorcourt.maricopa.gov" go to forms then look under guardianship/conservatorship for an adult; or you can obtain the same at 101 W. Jefferson, Phoenix, Arizona (Superior Court)
