Quit-Claim Deed
When we refinance our home, do we need to file a Quit-Claim Deed to put it back into our family trust?

Yes, by a deed like the one you signed to place it in the trust in the first place.

If you conveyed the property out of the trust to yourselves as individuals in order to refinance, then yes, you would need to convey the property by deed in order to put that asset back in the trust.
