Changing record title to devisee's name?

If X dies with a will and the will devises certain real property to Y, how does Y establish legal title to the property in Y's name? Assume the real property did not pass into the executor's hands and probate has been completed.

Should there be an order from the probate court distributing the real property to Y with a legal description of the property? Y is trying to figure out what needs to be recorded at the county recorder's office because title to the real property is still in X's name. Thank you.

How long ago and what county? There may be a need to re-open the probate, and transfer the property.
