which forms to file in San Diego, California to appeal a permanent civil harassment restraining order?

There is a standard Notice of Appeal form issued by the California Judicial Council. It's form APP-002, and it's probably available on the court's website. If not, should be easy to find online. You don't have to use the standard form, but it's easier than drafting your own and it's less likely to cause complications.

Note that there are strict filing deadlines which you must meet. An untimely Notice of Appeal will essentially doom your case.

Filing the Notice is the easy part. It merely gets the appeal started. There's a lot more to do afterwards, including the substantive briefs. You should get a lawyer if at all possible.
