I need to file a response to a summons (Credit Card Debt) and a pos form, right?

Q. 3 on the PLD-C-010 form states... do not check this box if verified. What exactly does that mean? Verified by who?

This is a summons from a law office on behalf of Capital One. They checked the; breach of contract, common counts and according to proof box for the; interest, attroneys fees and prejudgement interest. What will they have for all this according to proof?

I owe way more than $1.000. I do owe the debt but I am not sure which box to check.

Can you send me to a site that explains how/ how not to fill out this form?

I hear it is easy to fill out this form. But if a person is not knowledgable about law at all, it is not all that easy and is confusing.
