In responses to DISC-004 form interrogatories 102.2 should I forcible have to state place and date of birth or other statement. Question seems to me that plaintiff may profile or prejudge the defender

Yes. These form interrogatories have been reviewed by many courts that have found them to be legal. A party usually has an opinion of the opposing party anyway. Your information won't make a difference in that regard. Sometimes this information is helpful in determining if the responding party was truthful in documents completed for other purposes, or can help locate medical records, for example.
