Legal Question in Family Law in California

My daughter is visiting form Oklahoma. I reside in California where our custody was determined. Her mother has moved her 4 times in the last two years and has had 3 different men in my daughters life including a new baby but my ex is not with the father. When my daughter whom is 6 came last saturday I saw that her birthmark on her leg had recently been removed without my consent. We have Joint legal custody. My daughter then informed me that she is to have another surgery but that this time she will be put to sleep to they can cut her leg open and fix her scar. I ended up having to take her to the dentist yesterday because she had 7 cavities some which were on her adult teeth. Her mother has reported myself, my girlfriend, and my girlfriends 7 year old son to CPS when my daughter and her son were 4 and 5 claiming my daughter was raped by him. This was unfounded. She reported my girlfriend and I for general child neglect. Unfounded as well. My daughter has 3 visits a year with my in which over the last two years I have only seen my daughter twice. The other times her mother refused to allow the visit even though I had purchased plane tickets. Is this grounds for a custody change? I would like my daughter to come live with me since her mother is unstable. She told the courts the move to Oklahoma was for a job. She has never had a job out there and its been 2 years.

It certainly sounds like time for a visit to court requesting a change, to me.
