Where do I indicate a corporate defendant's agent for service of process on the Sum-100 Summons form??? Thank you!

I agree with Mr. McCormick. You have that document and the complaint served on the agent for service, but you don't fill it out. The box at the bottom is checked at time of service to indicate to the agent that they are served on behalf of a corporation.
