Legal Question in Personal Injury in California

If I sell a ladder to a friend and he injures himself while using it (i.e., the ladder fails either as a result of his negligence or due to an unknown defect), am I liable? What sort of document would be required to release me from any future liability?

I doubt you would be liable at all. You would only be liable if you sold it with know defects. The manufacturer would be on the hook for defects, known and unknown, but if you are not the manufacturer or retailer, why worry?
