No form; you need to contact a probate/trust litigation attorney to asist you in removal of the trustee, and compel an accounting of all of the assets of the trust.

The process for removing a trustee requires a formal, verified petition to remove the trustee. It also requires the formal presentation of evidence, and perhaps even a trial. It is not something a person with little training in the law can do by himself or herself.

I strongly suggest you contact a lawyer that can handle trust litigation to write a demand letter to the trustee and otherwise represent you in this matter.
