Which form should I use for a Will being I am in a domestic partnership that will soon be terminated? We have no children but do share custody of our pets.

I can only STRONGLY recmmend that you do not use a will that is a form. Additionally, if you desire to bypass the Probate process, you should consider having a trust drafted as well.
