Scandalous Trustees

What court form do I need to file to bring the trustees of my trust into court for failure to disclose acccounting of my trust?

If you don't know what you're doing, may I respectfully suggest you consult a lawyer instead of trying to do it yourself. You'll look silly, you'll lose, and you'll wind up being ordered to pay money to the villains.

As far as I am aware there is no canned form to do what you want, you would have to draft your own pleading and arrange to have it filed, served, and heard by the court.

There is no "form" for doing this. You will need to file a Petition for Accounting with the Court. Be aware, however, that the trustee has to have either (1) never provided an accounting, or (2) a beneficiary made a written request and no response, or an inadequate one, has been received for a period of three months.
