You have to prepare a deed conveying the property from yourself to the trust and then record the deed with the County Recorder's Office.

By deed. A quitclaim deed will do. You should not have to ask this question if you know what a trust is, isn't and how it works. You should consider reading my book for the lay public on the subject.
