Qualifications for health care insurance

Do I need to provide certification or proof to an insurance carrier, to obtain health insurance for a common law spouse? If so, what type of form and who can authorize it?

You will have to establish the fact of a common law marrige in the law. Just saying so won't do it. Unmarried partners cannot get health insurance with their partners in most instances.

Filing a declaration of common law marriage with the county clerk in the county in which you reside in a state which recognizes common law marriages is one answer.
