Guardianship of minor

My mother recently passed away (I am 30), and she left my minor sibling as beneficiary of her 401-K. Since she cannot touch money until she is 18, the retirement company needs proof of guardianship of my sister. That person is her father (my ex-step-dad) my mom and him are divorced, but he is her guardian, lives with her and takes care of her. What form does my stepdad need to fill-out to get this precess started?
