what does BOA-BASED ON AFFID INFO FILED mean. can someone explain it for me

I haven't seen that specific code, but I suspect that it just means the state filed a formal charge called an "Information." The state also filed an affidavit in support. However, you should talk to an attorney in your area to be sure.
