You need to file a Petition for Paternity, Visitation, Child Support and Custody. Until you file a court action there is not much you can do because she went through the Dept of Revenue and they only setup child support and nothing else.

You need to see a local attorney. If the child is receiving Medicaid, DOR will seek the full cost from you, additionally they will seek guideline child support regardless of the desires of the Mother. As to visitation / time-sharing DOR does not have authority to get into that, they only go after money. You need a local attorney and to file a paternity action to establish your rights.
