Legal Question
Where can I optain a form to submit to family law judge for motion for granting of a passport of a minor child? I am attempting to do this without paying attorneys fees/

Legal Answer
There is no real form for this. Just do a motion asking for a passport and explaining why you need it. Actually, I do not believe that you need any permission to get a passport. You may need permission to leave the country and certainly need permission if it will be a relocation of residency.
