Legal Question in Medical Malpractice in Florida

The legal form I personally can use to notify a nursing home I plan to take action even though the 2 year deadline for medical malpractice is next month.

There is no such form. There is a complaint that extends the statute by 90 days. You need and should have an attorney assist you in the presentation of this form.
