POA Health Form

Hi, once the form is filled out and notarized, what is to be done with the form? Is it just kept with the authorized 'agent' to be used as 'proof' later should it be needed? Please advise.

Hello. I presume that you are referring to the Living Will which gives instructions on what to do if the person lapses into a coma and does not wish that life saving measures be administered by the doctor. The doctor should have a copy of this form on file. At the

person's discretion, a copy of this instrument can also be given to a trusted family member. Will be happy
