Child custody and visitation

How do we go about setting up child custody and visitation in Illinois? My wife is from Ukraine and is legally in the USA. Her ex husband is also from Ukraine and is now legally in the USA and working as a interstate truck driver. We used to live in AZ and filed my wifes custody papers as a foreign judgement in Maricopa County. We are currently working with the Illinois States Attorneys office for child support. Her ex refuses to pay child support of any kind. He never has either. Recently he called and threatened to kill my wife. We are getting a restraining order. He lives in Beaverton Oregon. We tried many times to work in cooperation with him for visitation. He refuses to do what is right and since he threatened to kill my wife, she wants to have no visitation rights for him. Can she do this? How do we go about setting up these documents legally?

You need to have a judge decide the visitation issue. I would speak with the States Attorney and let him advise you as to what needs to be done.
