Is it possible for an Italian that hasn't a greencard to work as a volunteer?

what kind of visa do I need?

Unfortunately, there is not a non-immigrant visa category for volunteers. There are visas for trainees (H-3 or J-1) & professional, licensed interns (H-1B), however, if you are going to enter as a visitor, you are not supposed to do any kind work, even if it is unpaid.
