is a server responsible for paying the tab if a customer walks out?

Depends on the agreement between the server and the employer. Servers are generally employees at will so the owner can require this and fire the server for not paying. However, many restaurants cover skips and do not charge them to servers, which is the right policy since it is an inevitable part of doing business and the server may not have anything to do with the skip.
