Legal Question in Family Law in Indiana

I have full custody of my nine year old daughter her mom hasn't seen her or called her in over a year. Not to muchin that she hasn't gave me a dime ever what can I do to get sole custody of her I can't afford a lawyer so any advice and where to get the forms would be very helpful.

You have all the custody you will be awarded. There is no form of custody that will terminate the mother's rights to spend time with the child, which seems like what you want to get.
