My employer received a Child Support Information/Affidavit Form. Is my Ex trying to get more money? Or is this something that is just done?

There are two reasons why the Information Sheet might have been sent. If child support has not yet been established, then the form is needed to get income verification so child support can be calculated accurately. If child support has already been determined, then the form is normally used to get information needed to start income withholding or garnishment of wages. From the status of your case, you should be able to tell which of these situations is more likely.
