How do I file create a motion of reconsideration for maintenance awarded

I want to file a motion of reconsideration for maintenance that I have been ordered to pay by the court. My divorce decree was signed 10/27/06 and I understand I file 30 days to file this motion. Is there a form or template I can follow?

No there is no standard form unless your local clerk's office has one.
