We have legal, documented, grandparent visitation rights. Does the mother of the children legally have to notify the grandparents of address and phone number if she is moving?

Look at the Custody and Visitation Order. While grandparents can get visitation like an noncustodial parent, they may not have the same rights to residential information. If you have the right, it should be spelled out in the Order. You may have to file a request for injunctive relief to either prevent a move or get Notice and an opportunity to Contest.
