Legal Question in Wills and Trusts in Massachusetts

My mother has a will in the case of her death however if she is incapacitated and needs to go into a nursing home it is my understanding the State of Massachusetts will take and sell her home to pay for her care after insurance/Medicare/Medicaid, etc. runs out. It is my understanding that the home can be signed over at the time she is enrolled in a nursing home to a sibling who is legally disable and receiving disability. Is there specific paperwork I can keep on hand in case this scenario happens?
