Name Change
I need to know what form I need to use to get my son's last name changed to mine. I've never been married, but he has his putative father's last name and I want to change it.

You should go to the probate court in the county where you live to obtain the form. However, the putative father will have to be notified if your son is a minor, I believe. William S. Stern
