Legal Question in Criminal Law in Missouri

I just came across a agreement form for USDA that my boss had sined but then noticed in the seal of asset that my name was signed on that line and I didn't sign it. My signature was forged. They don't know I have a copy of this but this scares me what can I do if anything.

Decide upon the importance of what is claimed in the document. If you agree with statement, perhaps you do nothing. If you choose not to raise the issue with your employer, you might write to the USDA, and inform them that your name was inadvertantly listed as the signator on that form.
