If a man with no living relatives available to make medical decisions in case of an emergency is engaged to be married does this make his Bride to be next of kin prior to the marriage?

No. But, if there are no relatives to the 6th degree, a judge might allow the fiance to have some say. If the gentleman wants, he can appoint his fianced to be his attorney in fact, That would give her the say so over even close relatives.
