i recently gave my 2 week notice, a week before my last day i got a back injury. im now going through physical therapy. having problems finding a new job since im on doctors restrictions. does the company that ive worked for for the past 2 yrs. still have too pay disability since i was injured before my last day.

Your employer would still be responsible in providing your workmen's compensation benefits.
It is immaterial if it is your first day or last day on the job, an employer is responsible as long as your injury occurred within the course and scope of your employment.
