Who is responsible for an invoice not sent to a right address?

In June of 2006, I suffered a seizure type trauma in Oregon visiting a friend. The local FD gave me an ambulance ride to the nearest hospital. The charges were over $1200. I never received an invoice for the ambulance ride, but I eventually was contacted by a collections agency about it. The FD apparently got my address from my driver's license, which had my old address. They never (as I recall) asked me if the address on my driver's license was my correct billing address. I have contacted the FD and they sent me forms to fill out regarding my insurance. However, there are interest fees of $107.65 included. (.015% of the total amount.) Now, with insurance, the cost of services should be only about $400. Am I responsible for this interest when they never successfully contacted me with an invoice? They had my phone number, but they never once called me. Basically, is it my responsibility to pay an invoice that never got to me?

If they provided a service, you have to pay for it. If you had medical insurance, submit it to them. If you did not, call them and explain the situation. They should work with you on this, but you may need to get a supervisor to help you out.
