My Sister passed away and had no estate. She has an annuity to be divided 4 ways each month until 2012. She left behind many medical and credit card bills. Can a bill collectors go after the Annuity?

This is going to depend on how the annuity is set up. If her estate has a claim to it, then yes, the bill collectors can go after it. If her estate has no claim to it and it goes directly to the 4 recipients, then they probably cannot reach it. I notice you titled your question "trust". If a trust is involved, it may also depend upon the way the trust is set up. See an attorney to get specific details for your situation.
