waiver to award claim to another party
we would like ask for assistance what form or document to prepare for a state insurance claim which our company has shouldered because our truck has created the mishap resulting to death. since the beneficiaries has the right for the death benefit claim, we voluntarily waive the right for their beneficiaries to claim. can you assist us what to prepare. thank you.
