What froms do I need to file to have old charges removed from my record .

Here is the form used if the old charge was dismissed, if you were found not guilty, if the grand jury returned a finding of no true bill; or if you successfully completed pretrial or post trial diversion. It is also the form used for certain juvenile court matters that may be lawfully expunged from your record.

These forms are available at the Clerk of the Court's office. The Clerks are generally very helpful in assisting you in filling out the form and having the judge sign them. There is a fee for diversions, but no cost for dismissal/not guilty/etc.

If you are seeking the expungement of an offense that resulted in a final conviction (that is, you were in fact found guilty of the offense) then different rules (and forms) apply. In short, it is more complicated and costs more money.
