How do I ammend my Texas birth certificate after I change my name?
I am changing my last name in Ohio
(as I have been a resident for 12+
years), but I haven't been able to
find information on how to
amend/append the new last name
to my Texas birth certificate.

If you visit the website for the Texas Department of Health Bureau of Vital Statistics, you will see that they have a section regarding amendments to birth certificates. Good luck.
