child custody

my daughter is 15yrs old and has decided she wants to live with her dad. I have sole custody of her, and since she had decided she waants to live w/ her dad he and I are both in agreement of that and would like to do this through the court what do we need to file for this.

You and your ex need to file a Motion to Modify, together with an Agreed Order of Modification. If he has been paying child support, he will probably also want a Termination Order to stop withholding. There is a small filing fee of $20, and the court may approve the order without a hearing in some cases. Please contact me if I can be of any further assistance. TCL
