Name change 20 years after divorce

I chose the wrong form and I don't know how to get out of the program and into the more appropriate one

I am not sure what you are trying to do, but if your primary purpose is to change your name, the simplest is to do it in district court. The basic cost is $100, and you should be able to do it yourself. Visit this site for forms: http://www.metrokc.gov/kcdc/ncinfo.htm, you fill it out and pay the filing fee when you file it. Also at this site you can find the schedule for name change hearings. Based on your zipcode the nearest King County District Court Division is probably the one at:

Shoreline, WA 98133

The big job will be getting the changes registered with all the agencies and businesses.
