There is no format for reply to a notice. The reply of the notice should be based on actual facts and if possible, based on available documents.

Dear Fellow Attorney,

Your client should give the break-up of all the charges in the bill along with the number of days stay of the patient in the hospital. Consultation fees of other specialist doctors like skin specialists etc., to routinely check on the patient for prescribing the line of treatment also needs to be included in the bill. Lastly, you must also provide a photocopy of the 50% payment receipt that your client has issued to the firm owner in your reply to the notice.
