hi,im jen may i ask po sa case ng anak ko pinanganak po cia year 1995 hindi po kami kasal nun father nya until now gusto po sana nya magamit un surname nun father paano po ba nag proseso and mga magkano po yun aabutin sa change surname thanks

GET AN AFFIDAVIT TO USE FATHER'S SURNAME. HAVE IT FILED WITH THE LOCAL CIVIL REGISTRAR OF CITY OR MUNICIPALITY WHERE YOU FILED HIS CERTIFICATE OF LIVE BIRTH. THERE IS FORM AND THE LISTS OF REQUIREMENTS.
