Child Custody Enforcement needs Change of Custody

A father has had physical cutody of minor child since 1998, yet IRS has siezed all refunds for past child support for this child to his ex. Child custody enforcement wants a change of Custody form so that they can let go of the lien on him. Can you tell me where to find the form and any helpful legal advise?
