Permission to relocate

Hello, in seeking permission from the court to relocate my minor child to another state, is there a particular form that needs to be filed with the court? Also is there a particular form that I must send to my ex-husband notifying him of my intent or do I just send him a hand written letter certified? We share joint custody, however, I have physical custody of him 90% of the time.
