I need to file an ''Opposition Form'' but I can't figure / find which is the right form. Can you tell me the name of the form or the form number?

Generally, oppositions are pleadings, so you have to create your own. Check the law library at or near your local courthouse for reference books or, better yet, consult an attorney.
