An attorney would have to review your case to fill out the form. Are you in pro per, do you want to remain in pro per? One cannot help you publicly on this board, you'd have to privately confer with an attorney. If you want, contact me privately to review the matter. You also may have a FDCPA or CA FDCPA claim in this matter. An attorney would have to review your papers to determine that.

You should persue the debt validation issue. There may be a violation of the Fair Debt Collection Act. We would have to review your documents, but maybe can help you.
